The American College of Radiology (ACR) in December 2025 released the latest edition of BI-RADS, the definitive guidelines used by breast radiologists across the U.S.
The many changes in BI-RADS version 2025 include new information and over 900 clinical images that serve as a reference for breast imagers. These include updates to mammography, ultrasound, MRI, and contrast imaging, as well as auditing and outcomes monitoring.
Stamatia Destounis, MD, a co-author for the now-current BI-RADS, joins the show to discuss what led the ACR to bring experts together to make these updates. She also highlights the move to make BI-RADS a digital resource, including how this will make way for more timely adjustments in the future.

Destounis is a fellowship trained breast radiologist at Elizabeth Wende Breast Care in Rochester, NY, and has been practicing for 33 years. She is a fellow of the ACR, the Society of Breast Imaging, and the American Institute of Ultrasound in Medicine. She also currently serves as Chair of the ACR Commission on Breast Imaging.
Destounis also discusses what she's looking forward to for spring conferences in 2026 and how AI may play a role in future editions of BI-RADS.
