Are CT lung screening trials underestimating harms?
The complication rates and subsequent costs of invasive diagnostic procedures ordered after positive CT lung screening exams were two times greater in clinical practice than reported in the National Lung Screening Trial, according to an article published online January 14 in JAMA Internal Medicine.

Risk-targeted incentives may bolster CT lung screening
CT lung cancer screening programs that offer incentives for screening individuals with high mortality risk may not only boost participation rates but also could lead to considerable health and financial gains on a population level, according to an article published in the January issue of Health Affairs.

CT CAC reveals high heart disease risk in South Asian men
The incidence and progression rates of coronary artery calcium (CAC) on the CT scans of South Asian men living in the U.S. were as high as those of whites and markedly higher than those of black, Chinese, and Latino men, according to an article published online January 11 in the Journal of the American Heart Association.

Is fluoroscopy or CT guidance best for spinal injections?
CT guidance allowed interventional radiologists to perform spinal injections just as effectively as they did using traditional fluoroscopy but with much lower personal radiation exposure, according to an article published online January 8 in Radiology. The cost? Increased radiation exposure to patients.

High prices drive healthcare spending in the U.S.
Per capita healthcare spending -- including for medical imaging -- is much higher in the U.S. than in any other country, and the spending rate is increasingly trending upward, according to a study published online January 7 in Health Affairs.

Expeditors can help improve emergency CT workflow
Could a staff member assigned to serve as a dedicated expeditor in the emergency department boost the efficiency of CT exams? Most assuredly, say researchers from New York, who found the new role helped reduce CT turnaround times by more than 20%. Their findings were published online in the Journal of the American College of Radiology.

Will new USPSTF review lead to CT colonography payment?
The U.S. Preventive Services Task Force (USPSTF) has opened a review of its recommendations for colorectal cancer screening. While the USPSTF already recommends the use of CT colonography for colon screening, proponents of the virtual exam hope the new review will prompt Medicare to pay for the exam -- which, so far, it has resisted.

Can CTC improve staging for high-risk colon cancer?
CT colonography (CTC) may be better than conventional CT for staging high-risk colon cancer, improving the identification of patients who could benefit from chemotherapy before undergoing surgery, according to research published in the January issue of the American Journal of Roentgenology.

Images of a 39-year-old who underwent screening mammography. Right craniocaudal (left) and mediolateral oblique (middle) views from preoperative mammography show suspicious grouped fine pleomorphic calcifications (arrows). After biopsy showed ductal carcinoma of situ (DCIS), patient underwent breast-conserving surgery, yielding final diagnosis of ductal carcinoma in situ (intermediate grade, hormone receptor positive). (Right) Screenshot of an output of a commercial AI tool applied to preoperative mammography. AI detected right breast lesion with score of 95% on both views (concentric colored circles). Present study’s reviewing radiologist deemed AI marking to correspond with site of DCIS.

Could AI scoring help with managing DCIS?
Axial contrast-enhanced CT scan in a 28-year-old premenopausal woman shows an incidentally detected solid-appearing left ovarian lesion (solid arrow) anterior to the uterus (U), associated with trace ascites in the right adnexa and cul de sac (dashed arrows). At pathologic analysis, this was shown to be ovarian dysgerminoma. Two readers misdiagnosed this lesion, one as a leiomyoma and one as an ovarian fibroma.

An example of a typical BgRT plan: A) PET scan on diagnostic PET/CT and B) X1 PET-Linac for patient #15 with a PET avid rib metastasis with GTV size= 3.1 cc, PTV = GTV + 5mm, BTZ (biological tracking zone) = PTV + 5 mm, AC = 14.33 kBq/mL and NTS = 21.04. Red, magenta, and green contours represent GTV, PTV, and BTZ. C) Dose distribution and D) bounded-DVH of a BgRT plan are displayed for the same patient. The bounded Dose-Volume Histogram (DVH) models variations in BgRT delivery, encompassing fluctuation in target background signal variation, target localization and dose delivery uncertainties.

The model’s point of interest is highlighted with red, yellow, and green. In both the upper and lower panels (E and H), the point of interest is located in a region around the upper esophagus that corresponds to an air esophagogram and/or air-fluid level in the esophagus in the barium esophagogram (D and G).
