Triad Radiology data breach affects more than 11K patients

Triad Radiology Associates (TRA), a Winston-Salem, NC-based radiology group, is notifying more than 11,000 patients of a data breach that occurred in mid-2025.

An unauthorized third party accessed an employee's email account between July 11 and July 30, 2025, according to TRA in a February 8, 2026, website notification. The U.S. Department of Health and Human Services Office for Civil Rights noted a breach submission date of February 6, 2026, and 11,011 patients affected.

TRA provides diagnostic imaging and interventional radiology services to a number of facilities located in the Piedmont Triad area of North Carolina and has partnerships with Novant Health, with additional presence at Ashe Memorial Hospital, Life Bright Hospital, Northern Regional Hospital, and others, according to information about the group.

The practice discovered suspicious activity on July 30, 2025, and engaged a third-party specialist to investigate. The review concluded on December 10, 2025.

For some affected patients, the compromised information may be limited to first and last names in combination with a Social Security number. However, the type of information varies by individual and may include bank account information, medical information, and health insurance information, the practice noted.

TRA said it has no evidence that any information has been or will be fraudulently misused, but affected individuals are being offered 12 months of free credit monitoring and identity protection services.
