Triad Radiology Associates (TRA) of Winston-Salem, NC, has disclosed a data security incident in which an unauthorized third party may have accessed protected health information through an employee email account.
TRA said it discovered suspicious activity on or around July 30, 2025, and confirmed on December 10, 2025 that a limited amount of personal information may have been accessible. On February 6, TRA provided notice via a posting on its website and on February 11 began providing direct notice to impacted individuals.
The affected information varies by individual and may include names, addresses, Social Security numbers, driver's license numbers, bank account information, dates of birth, medical information, and health insurance information, the company said. Some of the information belonged to patients of other medical providers for which TRA provides radiology services.
TRA said it has no evidence that the information has been viewed or misused. The firm is offering complimentary credit monitoring to potentially affected individuals and has notified the U.S. Department of Health and Human Services Office for Civil Rights, according to the announcement.
