Classic PCP:

The HIV positive patient shown below presented with rapidly progressive shortness of breath. CXR demonstrated characteristic findings of PCP with bilateral perihilar interstitial infiltrates. These can rapidly progress to diffuse air space consolidation over 3 to 5 days. (Click image for coned view)
