CMS makes no change to coronary CTA payments
In a victory for advocates of coronary CT angiography (CTA), the U.S. Centers for Medicare and Medicaid Services (CMS) today decided to make no change to the way it reimburses for coronary CTA exams. With the decision, CT advocates have dodged a bullet that many feared could have killed the promising new field of cardiac imaging with CT.

Prospective gating drops cardiac CT radiation dose
VIENNA - Prospective gating has emerged as a promising method for reducing radiation dose in coronary CT angiography (CTA) studies. Monday at the European Congress of Radiology (ECR), Italian researchers confirmed previous results in using prospective gating to reduce dose to as little as 2.8 mSv per heart study, while a German group discussed its effort to further refine prospective gating by making it more accurate.

Mobile DR helps cut ED image transmission times
VIENNA - Using a mobile digital radiography (DR) system in the emergency department (ED) ward helped a radiology department at a French hospital cut the amount of time to image ED patients in half, according to a presentation on Sunday at the 2008 European Congress of Radiology. But the department's efforts to make the efficiency gains permanent have been stymied by government regulations on radiation exposure in healthcare facilities.

CAD provides mixed benefits for DR lung exams
VIENNA - A pair of presentations on Saturday at the 2008 European Congress of Radiology on computer-aided detection (CAD) for digital radiography (DR) reported mixed benefits for the software in detecting lung nodules in thoracic exams. The presenters discussed their experiences with a commercially available CAD product and its impact on radiologist performance in analyzing DR lung studies.

ECR sessions examine DBT's effectiveness
VIENNA - A series of presentations on digital breast tomosynthesis (DBT) at the 2008 European Congress of Radiology (ECR) reported encouraging results with the technology. Clinical investigators shared their experiences with DBT systems from four different manufacturers, and reported on the advantages of DBT compared to conventional full-field digital mammography (FFDM).

Study: Picking US over MRI for MSK imaging could save billions
The U.S. could save up to $736 million annually -- and billions over the course of the next 15 years -- if physicians used ultrasound rather than MRI for musculoskeletal imaging when clinically indicated, according to researchers from Thomas Jefferson University in Philadelphia. They examined how much money could be saved if ultrasound were substituted for MRI for musculoskeletal imaging applications in which clinical evidence shows that ultrasound is at least comparable to MRI.

Active thyroid surveillance effective, beneficial for older patients
A 58-year-old woman was diagnosed with ER-positive (Allred score 8) ILC in her right breast. Ultrasound and MRI found prominent lymph nodes in right axillary level I area, diagnosed as metastasis on ultrasound-guided fine-needle aspiration. F-18 FES-PET/CT showed a positive mass in right breast (white arrow; SUVmax, 5.6) and multiple F-18 FES–positive lymph nodes in right axillary level I, II, and II/III junction areas (red arrows; SUVmax, up to 12.2). In addition, a small F-18 FES-positive lymph node was observed in the right second internal mammary area (red circles; SUVmax, 1.8). However, biopsy could not be performed because it was too small and not visible on ultrasound; the clinician decided to include internal mammary lymph node in treatment field for adjuvant radiotherapy. The patient underwent lumpectomy with ALN dissection, with five metastases being identified in 13 lymph nodes dissected, and is scheduled to receive adjuvant chemotherapy, radiotherapy, and endocrine therapy.

FES-PET effective for staging breast cancer in phase II trial
By Will Morton
F-18 fluroestradiol (FES)-PET/CT appears as effective as standard imaging for staging women with estrogen receptor-positive locally advanced breast cancer or evaluating suspected recurrence.
July 26, 2024
The patient was a woman in her 50s with suspected recurrence of invasive lobular breast cancer. A, maximum intensity projection image from FDG-PET/CT demonstrating avid foci in the chest (red arrow). Transaxial CT (B) and transaxial fused FDG-PET/CT image (C) showing that the chest avidity corresponds to FDG-avid lung nodules (red arrows) suspicious for malignancy. The lung nodule was subsequently biopsied but found to represent benign granulomatous inflammation and thus a false positive on FDG-PET/CT. D, maximum intensity projection image from FES-PET/CT demonstrating multifocal uptake suspicious for malignancy. E, transaxial CT and fused FES-PET/CT demonstrating no FES-avidity in the biopsy proven benign granulomatous lung nodules (red arrows), thus true negative on FES-PET/CT. FES-avidity in nodes (white arrow) is suspicious for malignancy. F, transaxial CT and fused FES-PET/CT demonstrates gastric avidity (blue arrow) suspicious for malignancy. G, transaxial CT and fused FES PET/CT demonstrates FES-avid osseous foci (yellow arrow) suspicious for malignancy. This osseous focus was subsequently biopsied and proved to be an osseous metastasis and thus true positive on FES-PET/CT.

CT images from a 24-year-old woman with history of ventriculopleural shunt placement who presented with chest pain show two small linear hyperattenuating structures (A, arrows), with larger hyperattenuating structure oriented parallel and very close to the rib. The AI algorithm interpreted the finding as a rib fracture. (B) An additional axial image from same exam indicates that the finding relates to the patient’s ventriculopleural shunt (arrow), passing alongside the rib. The radiologist made a correct interpretation upon assessment of the entire exam. Image courtesy of the ARRS.

PET/MRI rules out tau protein in late-life depression

A PET/MRI study has provided insights into the neurobiology of late-life depression, finding tau protein is not involved in the condition.

Coronal and axial slice of the average cortical F-18 MK-6240 SUVR [standard uptake value ratio] image for each study population. SUVR images were corrected for partial volume effects and masked with the gray matter mask used for volume of interest delineation. Overall tau accumulation is comparable between the late-life depression (LLD, middle column) and non-depressed cognitively unimpaired comparison (HC, left column) groups, while abnormal tau accumulation, which is most pronounced in the mesial temporal lobe, can be seen in the mild cognitive impairment due to Alzheimer's disease group (right column).
