CT spot sign predicts hematoma expansion in stroke patients

In patients with intracerebral hemorrhage (ICH), hematoma expansion in the first critical hours after symptom onset is associated with significantly poorer outcomes. Thanks to CT imaging and a new drug in the pipeline, it may be possible to identify and treat ICH patients at risk of hematoma expansion while there is time to improve outcomes, according to two new studies.

ASPECTS stroke scoring improved with perfusion CT

The Alberta Stroke Program Early CT Score (ASPECTS) rates the presence or absence of ischemia in 10 regions of the brain. Paired with noncontrast CT when a patient presents with stroke symptoms, ASPECTS has been shown to effectively predict which patients will benefit from thrombolytic therapy. A recent study using perfusion CT has found that ASPECT increases the accuracy for predicting radiological and clinical outcomes over noncontrast CT.

Relative threshold method correlates CT, MRI stroke infarct assessment

SAN FRANCISCO - Researchers at the 2007 International Stroke Conference are making progress toward standardizing imaging and clinical criteria for the optimal treatment of stroke patients. The preliminary results from a study by Boston researchers are good news for CT perfusion imaging, which is faster and generally more available than MRI to handle emergent stroke cases.

BrainLab wins patent victory

Image-guided therapy firm BrainLab of Munich, Germany, reported that the U.S. District Court in Denver dismissed a patent infringement lawsuit earlier this month that was brought by interventional technology firm Medtronic of Minneapolis and that the U.S. Court of Appeals for the Federal Circuit has upheld the decision.

MRI should be preferred test for suspected acute stroke

MRI should replace CT as the first-line test for accurate diagnosis of suspected acute stroke, according to a study published today in the
Lancet
. The authors found that MRI was superior to CT for diagnosing ischemic stroke, as well as acute and chronic hemorrhage. While cost is an issue with MRI, its greater diagnostic accuracy would justify the extra expense, they said.

SPECT/CT enhances octreotide imaging

Indium-111-labeled octreotide imaging has shown greater accuracy than CT or MRI scans in detecting somatostatin receptor-positive neuroendocrine tumors. But anatomic localization of these tumors solely based on octreotide scans is problematic, so these procedures are usually performed with CT or MRI. A recent study suggests that SPECT/CT in octreotide imaging can provide important diagnostic information to clinicians that is unavailable with planar whole-body scans.
